A Brief Introduction To Rheumatoid Arthritis
By George Harper
This is according to the National Institute of and Musculoskeletal and Skin Diseases.

You might not know it, but you could be suffering from rheumatoid arthritis. Although rheumatoid is commonly associated with the older generation (people over the age of 65), the disease is found in younger generations - even including children. How could you tell if you have it? Well if you're experiencing morning stiffness for no apparent reason (like if you had done a lot of strenuous exercise the night before, for example), you might be suffering from one of it's symptoms. As mild as you might think morning stiffness is, you really ought to give it some serious thought and consult with your doctor because if that morning stiffness is related to rheumatoid arthritis, you can work to prevent it from disabling or crippling you later on down the road to a point where you can barely function. But rheumatoid isn't just a physical condition. It has the propensity to tax your mental and emotional state of well-being tpp.

This is because can change the way you work, the way you interact with your family, and the way you entertain yourself with recreational activities. You might even know someone with rheumatoid and have observed how this disease changed not only his or her mobility, but also his or her outlook on life. Those of us without rheumatoid tend to take our ability to move anyway we want for granted, but when that ability slowly disappears right before our eyes, it's no surprise that we get depressed about it.

But it doesn't necessary have to be that way. With proper medications, education, support, and prescribed exercises, you could work to prevent the most severe forms of the disease - or at least prolong the worst case symptoms.

Arthritis works in two ways. First, it inflames the muscles, ligaments, and cartilage that sit in-between joints. And it's this inflammation that causes the pain,

Local leptin production in osteoarthritis subchondral osteoblasts may be responsible for their abnormal phenotypic expression
Conclusions: These results suggest that abnormal production of leptin by OA Ob could be responsible, in part, for the elevated levels of ALP, OC, collagen type 1 and TGF-beta1 observed in these cells compared to normal. Leptin also stimulated cell proliferation, and Erk 1/2 and p38 signaling. Taken together, these data suggest leptin could contribute to abnormal osteoblast function in OA. Is Phytalgic(R) a goldmine for osteoarthritis patients or is there something fishy about this nutraceutical? A summary of findings and risk of bias assessment
A food supplement containing fish oils, urtica dioica, Zinc, and Vitamin E (Phytalgic(R)) for osteoarthritis (OA) has now been tested in a placebo-controlled trial for three months and according to the authors has a very large clinical effect, considerably larger than any other known product. Even experts endorsing nutraceuticals for OA symptoms would probably agree that a nutraceutical with an effect size above .5 is rarely seen. Despite our concern about the trial registration taking place after the study was completed, and the likelihood that patients would note the taste of fish, thus leading to detection bias, we consider these data promising although with a high risk of bias. A case of gouty arthritis following percutaneous radiofrequency ablation for hepatocellular carcinoma.
We describe a case of acute gouty arthritis in a 71-year-old man with chronic renal failure who was treated with RFA for a HCC lesion and who had hepatitis B-associated cirrhosis and mild renal insufficiency. Regular surveillance of the patient detected a 3.5 cm HCC lesion. Because the patient had declined surgery, RFA was chosen for therapy. On the third post-procedural day, the laboratory results showed increases in his uric acid and potassium levels, which were compatible with a tumor lysis syndrome. On the 6th post-procedural day, the patient complained of new right knee pain. Subsequent joint aspiration revealed monosodium urate monohydrate crystals. We made the diagnosis of acute gouty arthritis arising from tumor lysis and liver infarction caused by HCC ablation, which was aggravate...

A Variant of TNFR2-Fc Fusion Protein Exhibits Improved Efficacy in Treating Experimental Rheumatoid Arthritis
Author Summary Rheumatoid arthritis (RA) is a frequently occurring, chronic, debilitating disease. TNF? plays a pivotal role in regulating its inflammatory response. TNF? inhibition with TNF receptor 2-Fc fusion protein (TNFR2-Fc) was effective in the treatment of RA. However, quite a few patients may not achieve good clinical outcomes after TNFR2-Fc therapy, and the high dosage of TNFR2-Fc in clinical treatment usually causes some side effects, such as injection site reactions. Enhancing the affinity of TNFR2-Fc to TNF? would be of benefit to its therapeutic effect on RA, and may reduce the clinical dosage. We modeled the interactions of TNF? and TNFR2, and figured out the sites that may be critical for ligand-receptor binding. A high affinity TNFR2-Fc variant (E92N/W89Y) was obtaine...

Can Blood Samples Predict Arthritic Rheumatism?
Levels of inflammatory proteins, so-called cytokines, are elevated in the blood even before the onset of arthritic rheumatism. This means that such blood samples could be used to predict the development of the disease and thereby make it possible to prevent the pathological process, according to an article by Umea researcher Solbritt Rantapaa Dahlqvist and her associates in the journal Arthritis and Rheumatism. The research team analyzed blood samples from 86 individuals who donated samples to the Medical Biobank before they developed arthritic rheumatism... (Source: Health News from Medical News Today)

Potential Use of Plectranthus amboinicus in the Treatment of Rheumatoid Arthritis
Plectranthus amboinicus (P. amboinicus) is a folk herb that is used to treat inflammatory diseases or swelling symptoms in Taiwan. We investigated therapeutic efficacy of P. amboinicus in treating Rheumatoid Arthritis (RA) using collagen-induced arthritis animal model. Arthritis was induced in Lewis rats by immunization with bovine type II collagen. Serum anti-collagen IgG, IgM and C-reactive protein (CRP) were analyzed. To understand the inflammation condition of treated animals, production of TNF-, IL-6 and IL-1&beta; from peritoneal exudates cells (PEC) were also analyzed. P. amboinicus significantly inhibited the footpad swelling and arthritic symptoms in collagen-induced arthritic rats, while the serum anti-collagen IgM and CRP levels were consistently decreased. The production of pro...

FDA Approves Xiaflex For Debilitating Hand Condition
The U.S. Food and Drug Administration approved Xiaflex (collagenase clostridium histolyticum) as the first drug to treat a progressive hand disease known as Dupuytren's contracture, which can affect a person's ability to straighten and properly use their fingers. Dupuytren's contracture affects the connective tissue found beneath the skin in the palm of the hand. Too much collagen can build up, forming thick, rope-like cords of tissue that can prevent the fingers from being able to relax and straighten normally. The disorder is most common in Caucasians and in men over age 50... Comorbidities associated with psoriasis: An experience from the Middle East
Recent studies suggest that psoriasis patients have higher rates of comorbidities. We sought to determine the prevalence of comorbidities and co-medications in our psoriasis patients. We conducted case-control study in 1835 patients with psoriasis vulgaris and age- and gender-matched cohort without psoriasis. Patients were examined for clinical characteristics of psoriasis, PASI scores, and data of age, sex, body mass index (BMI), smoking status, comorbidities, and co-medications were analysed for both patients and controls. We identified 1661 (92.8%) patients with mild to moderate psoriasis (PASI < 10) and 129 patient's (7.03%) with severe psoriasis (PASI > 10). Patients with psoriasis were more likely to be current smokers (51.34% vs 32.51% controls). Respective prevalence rates of risk ...

HIV therapeutic possibilities of gold compounds.
Authors: Fonteh PN, Keter FK, Meyer D Highly active antiretroviral therapy (HAART) has resulted in decreased mortality and morbidity from the acquired immune deficiency syndrome caused by the human immunodeficiency virus (HIV). Drug resistance and toxicity of HAART has led to the search for novel inhibitors of HIV infection. Gold-based compounds have shown promising activity against a wide range of clinical conditions and microorganism infections including HIV-1. A typical example is auranofin which resulted in an elevated CD4+ T-cell count in an HIV patient being treated for psoriatic arthritis. In addition, reports exist on gold-based inhibitors of reverse transcriptase (RT), protease (PR) and viral entry of host cells. These and other characteristics of gold-based HIV drugs are revi...

Whiplash-associated disorders: who gets depressed? Who stays depressed?
Abstract&nbsp;&nbsp;Depression is common in whiplash-associated disorders (WAD). Our objectives were to identify factors associated with depressive symptomatology occurring in the initial stages of WAD, and to identify factors predicting the course of depressive symptoms. A population-based cohort of adults sustaining traffic-related WAD was followed at 6&nbsp;weeks, 3, 6, 9, and 12&nbsp;months. Baseline measures (assessed a median of 11&nbsp;days post-crash) included demographic and collision-related factors, prior health, and initial post-crash pain and symptoms. Depressive symptomatology was assessed at baseline and at each follow-up using the Centre for Epidemiological Studies Depression Scale (CES-D). We included only those who participated at all follow-ups (n&nbsp;=&nbsp;3,452;...

Suppression of LPS-induced inflammatory responses in macrophages infected with Leishmania
Conclusions: During uptake by macrophages, Leishmania evades the activation of a broad range of cytokines and chemokines. Further, in the presence of a strong inflammatory stimulus, Leishmania suppresses certain proinflammatory cytokine responses in a parasite-specific manner, however it augments the production of other proinflammatory cytokines. Our findings highlight the complexity of inflammatory cytokine signalling regulation in the context of the macrophage and Leishmania interaction and confirm the utility of the Leishmania/ macrophage infection model as an experimental system for further studies of inflammatory regulation. Such studies may advance the development of therapies against inflammatory disease. (Source: Journal of Inflammation)
Effect of cyclosporine on blood pressure
Source: Cochrane Library Area: Evidence > Medication Safety Abstract &nbsp; Background Cyclosporine is an immunosuppressive agent used for different autoimmune diseases. The official Canadian indications for cyclosporine are solid organ transplantation, bone marrow transplantation, psoriasis, rheumatoid arthritis and nephritic syndrome (e-CPS 2008). The expanding range of indications for cyclosporine therapy will lead to more patients receiving chronic therapy with possible side effects, hypertension being one of the most common. Therefore it is essential to know the magnitude of increase of blood pressure (BP) associated with cyclosporine in order to appropriately manage patients receiving the drug. &nbsp; Objectives The primary objective of this systematic review is to evaluate ...

Hsp40 proteins modulate humoral and cellular immune response in rheumatoid arthritis patients.
Authors: Tukaj S, Kotlarz A, Jozwik A, Smolenska Z, Bryl E, Witkowski JM, Lipinska B Recent research on the heat shock proteins (Hsps) in chronic inflammatory diseases indicates that Hsps may have disease-suppressive activities. Our aim was to characterize immune response directed to bacterial (DnaJ) and human Hsp40s in patients with rheumatoid arthritis (RA). We found elevated levels of anti-DnaJ, anti-Hdj2, and anti-Hdj3 (but not ant-Hdj1) serum antibodies in the RA patients (P &lt;/= 0.001) compared to healthy controls. In peripheral blood mononuclear cells (PBMCs) culture, all tested Hsp40 proteins significantly inhibited the divisions of CD4+ and CD8+ T cells of the RA patients but not those of the controls. Both DnaJ and Hdj2 stimulated secretion of the main anti-inflammatory cyt...
Fish Oil to Prevent Psychotic Disorders?
As reported by the Associated Press, this month the Archives of General Psychiatry has published the results of a study that was designed to find out whether omega-3 fatty acids such as are found in fish oil have any benefit in preventing psychosis from developing, specific in teens and young adults. The study was small - just 81 participants - but the results were strong enough to make the outcome very encouraging. The patients, aged 13 to 25, were selected on the basis of being at ultra-high risk for developing a psychotic disorder such as schizophrenia, based on early symptoms. After one year, 27.5% of the placebo group had developed full-blown psychosis, but only 4.9% of those taking omega-3 fatty acids had done so. If these findings stand up in larger studies, will the results turn ...

New 'suicide' molecule halts rheumatoid arthritis
Scientists have invented a novel way to halt and even reverse rheumatoid arthritis. They have developed an imitation of a suicide molecule that floats undetected into overactive immune cells responsible for the disease. Referred to as Casper the Ghost, the stealthy molecule causes the immune cells to self-destruct. The approach doesn't carry the health risks of current treatments. The new therapy, tested on mice, stopped the disease cold in 75 percent of the animals. Changes in the pattern of DNA methylation associate with twin discordance in systemic lupus erythematosus [LETTERS]
Monozygotic (MZ) twins are partially concordant for most complex diseases, including autoimmune disorders. Whereas phenotypic concordance can be used to study heritability, discordance suggests the role of non-genetic factors. In autoimmune diseases, environmentally driven epigenetic changes are thought to contribute to their etiology. Here we report the first high-throughput and candidate sequence analyses of DNA methylation to investigate discordance for autoimmune disease in twins. We used a cohort of MZ twins discordant for three diseases whose clinical signs often overlap: systemic lupus erythematosus (SLE), rheumatoid arthritis, and dermatomyositis. Only MZ twins discordant for SLE featured widespread changes in the DNA methylation status of a significant number of genes. Gene ontolo...

Free radical scavenging: a possible mechanism of action for the anti-inflammatory activity of Russelia equisetiformis (Schlect & Chan) (Scrophulariacae)
Abstract&nbsp;&nbsp; The methanol extract of Russelia equisetiformis (MRE), which exhibited significant antiinflammatory activity, was evaluated for the possible mode of action by studying its antioxidant potential in adjuvant- induced arthritic rats. 1, 1 diphenyl-2-picrylhydrazyl (DPPH) radical scavenging effect was determined by spectrophotometric method. The enzymatic activity of the superoxide dismutase, glutathione peroxidase, showed a significant increase, while the lipid peroxide content was found to decrease to a large extent in MRE and indomethacin-treated rats. There was also significant and concentration-dependent increase in the percentage inhibition of DPPH, comparable to that of vitamin E and butylated hydroxyanisole (BHA) which are natural and synthetic antioxidants, re...

From the item to the outcome: the promising prospects of PROMIS
Evaluation of patient reported outcomes, and in particular physical function, have gained increasing importance in research and therapy of patients with rheumatic diseases. Most instruments that are used for that purpose are rigid and suffer from floor and ceiling effects when used in patients whose physical function differs from the average. A new approach to the assessment of physical function is by computerised adaptive testing (CAT), by which precision and reliability of the measurement can be achieved for most patients, while even requiring less time for the assessment. Well calibrated and tested item and large item data banks are a prerequisite for this purpose, a process which is summarised in the present report by Bruce and colleagues. Outcome for juvenile idiopathic arthritis
Abstract: Juvenile Idiopathic Arthritis (JIA) is a heterogeneous group of chronic inflammatory conditions comprising several disease subtypes with a widely variable clinical course and outcome. An ideal treatment outcome is one where there is lifelong disease remission with no long-term functional or psychological impact. Although not always achieved, over the past decade there has been considerable progress towards this target. The quality of outcome for JIA is dependent on early diagnosis and intensive management to optimise disease control. This review looks at the key aspects of management in JIA that influence prognosis and outcome. (Source: Paediatrics and Child Health)
Transitional care for young people with connective tissue diseases
Abstract: Within rheumatology, much of the literature in transitional care has addressed young people with juvenile idiopathic arthritis with less attention being directed to another significant group, namely those with connective tissue diseases. These young people, although fewer in number, often present complex challenges during adolescence and the period of transition from paediatric to adult-centred services. This paper aims to explore the particular challenges transition presents to these young people and will discuss potential strategies which health professionals can adopt to effectively address their needs. Detection of multiple autoantibodies in patients with ankylosing spondylitis using nucleic acid programmable protein arrays.
Authors: Wright C, Sibani S, Trudgian D, Fischer R, Kessler B, Labaer J, Bowness P Ankylosing Spondylitis (AS) is a common, inflammatory rheumatic disease, which primarily affects the axial skeleton and is associated with sacroiliitis, uveitis and enthesitis. Unlike other autoimmune rheumatic diseases, such as rheumatoid arthritis or systemic lupus erythematosus, autoantibodies have not yet been reported to be a feature of AS. We therefore wished to determine if plasma from patients with AS contained autoantibodies and if so, characterize and quantify this response in comparison to patients with Rheumatoid Arthritis (RA) and healthy controls. Two high-density nucleic acid programmable protein arrays expressing a total of 3498 proteins were screened with plasma from 25 patients with AS,...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>
Measurement of quality of life and participant experience with the mindfulness-based stress reduction program.
The objective of this study was to collect quality-of-life data from subjects who participated in the first MBSR program offered at Mayo Clinic. The class was taught as a collaborative effort with the University of Minnesota that had an established MBSR program. Sixteen participants completed a validated, 12-question, linear analogue self-assessment instrument, administered at the beginning and end of the program. Comparison of assessment scores using paired t-tests showed statistically significant improvement in overall quality of life (P=0.04), mental well-being (P=0.005), physical well-being (P&lt;0.001), emotional well-being (P&lt;0.001), level of social activity (P=.02), and spiritual well-being (P=0.006). Although positive changes also were observed for frequency of pain, severity of...
KR-003048, a potent, orally active inhibitor of p38 mitogen-activated protein kinase.
Authors: Montalban AG, Boman E, Chang CD, Ceide SC, Dahl R, Dalesandro D, Delaet NG, Erb E, Ernst J, Gibbs A, Kahl J, Kessler L, Lundstr&#xF6;m J, Miller S, Nakanishi H, Roberts E, Saiah E, Sullivan R, Wang Z, Larson CJ The tumor necrosis factor-alpha (TNF-alpha) cytokine, secreted by activated monocytes/macrophages and T lymphocytes, is implicated in several diseases, including rheumatoid arthritis, chronic obstructive pulmonary disease, inflammatory bowel disease, and osteoporosis. Monocyte/macrophage production of TNF-alpha is largely driven by p38alpha mitogen activated protein kinase (MAP kinase), an intracellular soluble serine-threonine kinase. p38alpha MAP kinase is activated by growth factors, cellular stresses, and cytokines such as TNF-alpha and interleukin-l (IL-I). The pri...

Autoimmune rheumatic diseases: an introduction
The outlook for patients with autoimmune rheumatic diseases has improved. The past five years has seen a narrowing of the gap between an ever-expanding laboratory-based and clinical research arena, and the actual delivery of care at the bedside. Our knowledge about pathogenesis of rheumatoid arthritis, systemic lupus erythematosus, systemic sclerosis, the systemic vasculitides and other immunologically driven rheumatic diseases is being increasingly linked to intelligently designed therapies and diagnostic modalities. There is increasing awareness that, where possible, patients with these rare conditions should be treated in a research-oriented environment, for example, a clinical trial, so that this gap between the patient and laboratory continues to narrow. We are privileged to have lead...
Autoantibodies in the autoimmune rheumatic diseases
Abstract: The detection of autoantibodies to a variety of antigenic targets plays an important role in the diagnosis of autoimmune rheumatic disease. Rheumatoid factor, antinuclear antibodies and antineutrophil cytoplasmic antibodies have been the key in the diagnosis of rheumatoid arthritis (RA), systemic lupus erythematosus and systemic vasculitis for many years. Antibodies against cyclic citrullinated peptides have been shown to be specific for rheumatoid arthritis and may predict the development of erosive disease, and are being increasingly used in the assessment of patients with RA. There is increasing interest in the use of anti-C1q antibodies for the detection and monitoring of lupus nephritis as this assay appears to be more sensitive than anti-DNA antibodies. Antibodies against ?..<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

Most Patients Gain Weight After Getting A New Knee
You'd think folks who've had knee replacement surgery -- finally able to walk and exercise without pain -- would lose weight instead of put on pounds, but surprisingly that's not the case, according to a University of Delaware study. Researchers Joseph Zeni and Lynn Snyder-Mackler in the Department of Physical Therapy in UD's College of Health Sciences found that patients typically drop weight in the first few weeks after total knee arthroplasty (TKA), but then the number on the scale starts creeping upward, with an average weight gain of 14 pounds in two years... The genus Mansoa (Bignoniaceae): a source of organosulfur compounds
The genus Mansoa belongs to the family Bignoniaceae and it includes eleven species that occur mainly in the dry and wet forests of Brazil and from Argentina to the Southeast of Mexico. These species in the Brazilian Amazon region are known as "cipó-de-alho", that means garlic vine, in reference to the pungent garlic-like smell of the leaves when crushed. "Cipó-de-alho" has several uses in folk medicine and among them, the most cited are the treatment for cold, fever, pain and inflammation of arthritis and rheumatism. In spite of all those uses, it still has little application in phytotherapy when compared to garlic (Allium sativum). The essential oils of Mansoa spp. show the presence of allyl polysufides that contribute to the characteristic aroma and flavor. The chemical composition of ...

ADAM8 is a negative regulator of retinal neovascularization and of the growth of heterotopically injected tumor cells in mice
Abstract&nbsp;&nbsp;ADAM8 is a member of the ?a disintegrin and metalloproteinase? (ADAM) family of membrane-anchored metalloproteinases. ADAM8-deficient mice have no evident spontaneous developmental or pathological defects, and little is currently known about the role of ADAM8 in disease. Here, we investigated the contribution of ADAM8 to pathological neovascularization in mice using an oxygen-induced retinopathy (OIR) model and heterotopical injection of tumor cells. We found an increase in retinal re-vascularization but fewer neovascular tufts in the OIR model and increased growth of heterotopically injected tumor cells in Adam8-/- mice compared with wild-type controls. These results suggest that ADAM8 functions to limit both of these processes in wild-type mice. In cell-based...

Adult presentation of Stickler syndrome type III
We describe an adult presentation of the syndrome in a 67-year-old woman and provide a report on the clinical and radiographic features supporting diagnosis. A chart review and updated investigations were performed to elucidate the presenting history and disease progression in the patient. Clinically, the patient was 42&nbsp;years old when she presented with a 22-year history of bilateral knee pain and atypical osteoarthritis (OA) of an inflammatory but non-erosive character. The patient also reported increasing hearing loss for high-pitched frequencies. Radiography/X-rays at age 42&nbsp;years showed severe, non-erosive tri-compartmental OA changes. Atypical destructive changes were identified in the small joints. The son of the patient developed left knee pain and bilateral knee sw...

swelling, and heat. Those are symptoms that are typical indications of an injury and they're vital to understanding more about this disease. Second, works by releasing enzymes that basically consume or otherwise destroy the muscles, ligaments, and cartilage that have become inflamed to a point where they're not very useful and don't allow for easy movement. In the worse cases, cartilage disappears completely and as you can guess, this is extremely crippling and uncomfortable.

That's why we call rheumatoid a disease. Typically, inflamed muscles, ligaments, and cartilage are the result of an injury, like falling on the knees for example. But with arthritis, no injury has to occur. In fact, is a type of autoimmune disease and the cartilage inside joints is one of the things that it destroys. And any joint can be affected - one, two, maybe even more but most of the time, the disease targets fingers, hips, feet, and knees.
